Home / Practices
For medical practices and non-medical practitioners
If you consider blood sampling at home medically necessary for a patient, we support you.
This covers the entire process chain: appointment coordination, venous blood and sample collection by qualified professionals, temperature-controlled sample courier service and digital transmission of the laboratory results to you as the delegating doctor.

When and how to prescribe
Eligible for prescription in particular with
- severely restricted mobility
- no home visits available, or not in time
- an unreasonable practice visit (care level 3–5, disability markers “aG”, “Bl”, “H”)
Muster 10 + private prescription
- Laboratory parameters on Muster 10 as usual
- plus a private prescription for the mobile blood sampling
Laboratory request + private prescription
- Laboratory request as usual plus a private prescription
- alternatively, laboratory parameters and the order directly on the private prescription
The individual services (home visit, venous blood sampling, travel allowance, consultation, coordination) are standard EBM services (01410/01413, 40220/40222, 01435/01436/01647) and are listed in the GOÄ.
What is new is their integrated, cross-sector provision as a private service at a flat fee, in line with the targeted type of service under Section 140a SGB V.
What must be on it – and what must not
Must be on it
- Individual indication with its purpose, e.g. “clarification of …”, illness-related or follow-up check
- Reason on the patient’s side: a practice visit is unreasonable, e.g. care level, immobile due to serious illness
- Reason on the practice’s side: a home visit is not possible in time, e.g. for capacity reasons
Must not be on it
- “I don’t want to / cannot make a home visit”: the obligation to make home visits applies in principle. Only “not in time” is permissible.
- The name Creasano or any other provider: because of the ban on patient referrals, the patient’s free choice of provider and the health insurer’s right to name a provider itself in the application for benefits in kind.
Sample private prescription
For patients with statutory (GKV) and private (PKV) insurance.
The prescription names only the service, not a specific provider.
Why a laboratory request and a private prescription?
“Mobile blood sampling in the home environment including laboratory courier service” is a combined, cross-sector service that is not part of the EBM benefits catalogue. You therefore issue a private prescription in addition to the laboratory request.
Next steps for patients with statutory insurance:
- Application for benefits in kindThe patient applies to their health insurer for the service, enclosing the laboratory request and the private prescription.
- Decision of the health insurerusually within 3 weeks, or within 5 weeks if the Medical Service is involved (Section 13(3a) SGB V). The insurer names and commissions a provider. If it cannot, the patient may procure the service themselves and apply for reimbursement (Section 13(3) SGB V).
- If urgentIf the laboratory results are needed “urgently” by the date stated in the private prescription, the service must be provided in good time before then.
If the health insurer cannot ensure this, the patient may procure the service themselves and apply for reimbursement (Section 13(3) SGB V).
And this is how it works for privately insured patients:
- Laboratory request and private prescriptionPrivately insured patients also need a laboratory request and a private prescription from you – separately or combined on one private prescription.
- Direct orderPrivately insured patients can commission Creasano directly, without having to submit an application for benefits in kind.

How we comply with the applicable rules
These principles reflect applicable law and do not create any new obligations.
Prescription and responsibility remain with the doctor
Indication, laboratory request, private prescription, delegation and results lie with the prescribing doctor. The professional acts only on the doctor’s medical orders and instructions. A delegation agreement for the individual case exists between the prescribing doctor and the professional. The practice receives a completion notice and a documentation report.
Section 15(1) MBO-Ä · Section 28(1) SGB V · German Medical Association delegation principles 2008Vetted medical professional
Every medical professional is individually vetted and approved by the medical director. They work according to the quality management manual and SOPs and do not make any treatment decisions of their own. In addition, they are vetted by the prescribing doctor who delegates the task.
BMV-Ä · Section 630f BGBFree choice of doctor and provider
The prescription does not name a provider. Only the patient decides whether to commission Creasano. Patients with statutory insurance submit an individual application for benefits in kind to their insurer.
Section 76 SGB V · Section 2(2) SGB V · Section 31 MBO-Ä · Section 73(7) SGB VNo fees between the practice and Creasano
No patient referrals in return for payment, no influence on the choice of provider. The ban on patient referrals is observed.
Section 31 MBO-Ä · Section 73(7) SGB V · Section 7 HWGData protection and confidentiality
Creasano only processes the information needed for the order, no diagnoses and no laboratory results. The laboratory results are sent by the laboratory directly to the doctor or can be accessed online there. The professionals are bound to confidentiality.
Art. 6(1)(b), Art. 9 GDPR · Section 630d BGB · Section 203(4) StGBProximity and availability
The appointment is coordinated with the practice. The doctor can be reached by phone during the sampling and can intervene on site within a reasonable time if necessary.
Medical duty of care · VG München M 16 K 21.870 · BayVGH 21 ZB 24.1347Sampling by doctors or non-medical practitioners
If a doctor or non-medical practitioner (Heilpraktiker) takes the blood sample, they act without delegation, as is legally customary.
Section 630f BGB · Section 630d BGB
Prescription, laboratory and results remain with the prescribing doctor; in an emergency, the prescribing doctor is informed immediately.
