For health insurers, public health authorities and associations
Mobile blood sampling closes a gap in care for housebound patients. Here you will find how the service is classified and our delegation structure.
Medically ordered, outside benefits in kind
Service
Mobile venous blood sampling in the home environment including laboratory courier service, as a single, cross-sector service. The starting point is a prescription from the treating GP or specialist with a medical justification.
Position in the benefits catalogue
The individual components are provided for as separate services in the EBM and GOÄ. The combination does not exist as a service of its own in the EBM; there is as yet no selective contract under Section 140a SGB V.
Prescription
The doctor issues a private prescription and documents the indication and the reasons for sampling at home: a home visit is not possible in time, a practice visit is unreasonable.
Patients with statutory insurance (GKV)
An application for benefits in kind to the health insurer is intended (Section 2(2) SGB V), not advance payment with subsequent reimbursement.
What makes delegation effective
Venous blood sampling is the practice of medicine (Section 1 HeilprG), but a medical service that can be delegated. The decisive factors are:
- Medical order in the individual case
- The measure can be delegated
- Qualification and selection of the professional
- Professional guidance
- Authority to give professional instructions that can actually be enforced
- Medical supervision with the possibility to intervene
Professional delegation (Section 28(1) SGB V) and the professional’s status under employment and social security law (Section 7 SGB IV) must be assessed separately.
Supervision and responsibility structure
- Responsible doctor
- The treating GP or specialist who knows the patient and issued the prescription
- Proximity
- On-site intervention possible within 30–60 minutes
- Availability
- By phone within 15 minutes; this complements proximity but does not replace it
- Capacity
- Supervision only for patients of the doctor’s own practice
- Qualification
- Individually vetted and approved by the medical director
- Quality management
- Quality management manual and SOPs, reviewed by the medical director
- Doctors / non-medical practitioners
- Act under their own professional responsibility, without delegation
- Emergency
- Emergency number 112, immediate notification of the prescribing doctor
- Preliminary check
- Every order is checked in advance for a medical indication and a prescription; without both, no assignment is arranged. A voluntary quality standard to relieve the healthcare system. Only the information needed for the order is processed, no diagnoses and no laboratory results
